Cna’s Have You Ever Forgot To Chart Something And What Happened?
forgot to chart someone’s output from a catheter yesterday. I am going to talk to someone at work in like 3 days when I go back about charting it then. I was just wondering if anyone else has forgotten before and if you got into trouble for it and /or if they let you chart it when you came in next. It was my first day working as a CNA on my own and I was very busy the whole day and forgot. The person had 900 cc’s of urine I know that, isn’t that a lot of urine the to empty or is that normal? I was a CNA for 4 years. I forgot things like that from time to time. Just explain to the charge nurse what happened. At my nursing home, we wrote it in later on the intake/output sheet.
You sound like a decent and diligent CNA and they should be glad to have you on staff.
900 mLs is an expected urine output for a foley catheter bag that gets emptied once a shift.

If I were you I would call the charge nurse and let them know the output ASAP. The sooner the better. The reason why is most patients are being monitored on what they drink and how much they put out (intake and output). If they have any kind of renal problems etc.. this is very important. Some of the time the doctors will want this information to evaluate current health issues that the patient can be having or future problems that can arise. You should not get in trouble if it is taken care of right away, this next shift will need to know in case of changes in the patient. The longer you wait the worst it can be especially if something happens with the patient and needs to go the the ER. Good luck, hope that this helps.

You need to either call your manager and see if they want you to come chart it now or if it can wait until you come back from your days off. Your patient had 900ml out and it doesn’t matter if you think that is alot or not; its what you emptied and its got to be charted. In medicine, this is no “normal” anything and comparing one patient to another will only get you into a mess.
A big part of working in the any aspect of the nursing field is responsibility and accountability. That’s why this needs to be addressed. For all you know, the doctor had ordered strict I&O’s on this patient and by you not charting that 900ml, it could cause a problem. Do the right thing.
You can keep asking until you get a different answer, but having been a registered nurse for 20 years, I can tell you I’m the best most accurate response you’re going to get.
